Grade 7 Immunizations:
HPPH will be in schools this fall to offer the Hepatitis B (HB), Human Papillomavirus (HPV), and Meningococcal (Men-C-ACYW-135) vaccines to grade 7 students.
Student information packages for the grade 7 classes, will be provided in September or October to your school depending on your school clinic date. Information packages will include a parent letter, vaccine fact sheets and the consent form. HPPH will be collecting the completed consents prior to the clinic.
Please see attached calendar for your school's immunization clinic dates. Please confirm your school’s clinic date by return e-mail by Friday, September 1 so that our team can schedule staff accordingly.

To ensure that the immunization clinics run smoothly, there are a few things that we require:
- A room large enough for 3 - 4 nurses to provide vaccines and students to wait 15 minutes after vaccination (library, gym, classroom), along with our equipment and supplies.
Please have the following items ready in the school clinic space:
- approximately 8 chairs
- 2 - 3 long tables
- 3 - 4 garbage cans
- 2 gym mats
- Please ensure the information package is distributed to each eligible student in your school. If for any reason parents wish to change their child’s consent, it must be in writing.
- HPPH requests a copy of the grade 7 class lists. We will pick up these lists when consent forms are picked up.
- The consent return date is printed on the parent letter. Please encourage eligible students to return their consent forms by that date. An HPPH staff member will be in to collect the completed forms after the return date.
- Clinics may have to run over the nutrition break or recess time.
Additional Grade 7 clinic information:
